Asuhan Gizi pada Hipertensi Stadium II, Penyakit Jantung Hipertensi, Kolelitiasis, dan Perdarahan Uterus Abnormal

Abstrak

Penatalaksanaan multimorbiditas pada pasien geriatri menghadirkan tantangan klinis yang kompleks akibat adanya kebutuhan diet terapeutik yang saling bertentangan. Laporan kasus ini mengevaluasi efektivitas terapi gizi medis terintegrasi pada seorang perempuan berusia 70 tahun yang didiagnosis dengan hipertensi stadium II, hypertensive heart disease (HHD), kolelitiasis, dan perdarahan uterus abnormal (PUA). Intervensi gizi yang diberikan menyelaraskan diet jantung dan diet DASH dengan restriksi natrium ketat (~1.500 mg/hari) serta prinsip rendah lemak (20% dari energi total) guna memitigasi beban hemodinamik dan stasis bilier, sembari memastikan asupan protein bernilai biologis tinggi yang adekuat (1,3 g/kg). Luaran setelah intervensi menunjukkan perbaikan klinis yang signifikan dalam waktu 72 jam. Tekanan darah menurun secara drastis dari 160/90 mmHg menjadi 110/80 mmHg, dan kolik bilier mengalami resolusi total. Lebih lanjut, stabilitas hemodinamik tetap terjaga tanpa memperburuk risiko anemia meskipun terdapat perdarahan uterus aktif. Studi ini menyimpulkan bahwa strategi diet terpersonalisasi yang terintegrasi dengan terapi medis dan multitarget mampu memutus rantai patofisiologis multimorbiditas degeneratif secara efektif, sekaligus menegaskan peran penting gizi klinis sebagai pendekatan terapeutik noninvasif utama bagi pasien lansia berisiko tinggi.
